About the Role:
Greenhouse Pharmacy is looking for a warm, organized, and healthcare-savvy Community Care Coordinator to join our growing team. This hybrid role supports both patients and partner providers by ensuring smooth prescription onboarding, personalized follow-ups, and proactive care outreach. You’ll be the bridge between our pharmacy team and the people we serve—helping us deliver care that feels human, not transactional.
Key Responsibilities:
- Onboard new patients and ensure a smooth medication start process
- Coordinate with prescribers and clinics for accurate and timely script fulfillment
- Provide friendly, informative follow-ups via phone, text, or in-person
- Support marketing or outreach events in the Houston area as needed
- Document interactions with clarity and empathy
Qualifications:
- Background in healthcare, pharmacy, or customer care
- Strong communication and problem-solving skills
- Bilingual a plus (especially Spanish)
- Passion for patient-first service
